Job Purpose

The position is responsible for assisting the Day Habilitation Program Supervisor in all aspects of departmental functions on a daily basis. The position serves as both administrative support, as well as, program facilitation support.

Responsibilities

  • Assist the department supervisor in all aspects of the day to day activities
  • Assist in the direction of department staff
  • Assist in required administrative and state reporting
  • Submit Monthly Department attendance paperwork
  • Utilize Electronic Health Record System to create accurate and timely notes on all services delivered and relevant client information that meets or exceeds agency and state standards.
  • Correct all documentation as directed by supervisor.
  • Attend all electronic health record (Foothold) training as required.

Essential Functions

  • Open Day Habilitation Program building and set up for the day’s activity
  • Assist Day Habilitation Program Supervisor with daily tasks
  • Report and document any incidents/situations that occur when Supervisor is not present
  • Facilitate several groups within the Day Habilitation Program
  • Cooperates with the licensee or DDD staff in any inspection or investigation
  • Must check email and voicemail throughout the day on a daily basis
  • Complete monthly DDD paperwork
  • Meets with potential new clients along with the Day Habilitation Supervisor and files in for client orientation meeting when Supervisor is unavailable.
  • Provide Crisis Intervention as needed
  • Serve as liaison between clients’ and group facilitator
  • Participate in all staff meetings, trainings and client meetings as assigned.
  • Understand and follow client’s Individual Service Plan to ensure excellent service provision; provide assistance to program participants as needed.
  • Transport clients as assigned (able to drive a 15-passenger van).
  • Submit Monthly Department attendance paperwork
  • Foster the growth of individuals by teaching and offering opportunities for choice, initiation and risk taking.
  • Utilize Electronic Health Record System to create accurate and timely notes on all services delivered and relevant client information that meets or exceeds agency and state standards.
  • Correct all documentation as directed by supervisor
  • Attend all electronic health record (Foothold) training as required

Reporting

  • Assist in the preparation and monitoring of yearly Day Habilitation Budget
  • Assist in developing and maintaining of reports/metrics relative to service delivery and departmental performance
