Day Program Floor Facilitator

The Day Program Facilitator provides direct support to clients who participate in the Day Program. The Facilitator will create and facilitate groups that help clients achieve goals and outcomes identified in their Individual Service Plan. This position documents all information required to determine the client's progress towards the achievement of group and individual participant goals.
